Please look for blocks marked INSTALLATION NOTES, just like the one above.

The first note is this: make sure your regional settings (Control Panel -> Regional Settings) are “US”. This is most likely your case, but please check it, since CPRS will not work otherwise.

Disclaimer

The VistA-Office EHR (VOE) software and associated documentation are based on the existing VistA Freedom of Information Act (FOIA) software Application and documentation from the Veterans Health Administration. In the software and the associated documentation, there may be references to Veterans Administration, Veterans Health Administration, in-patient care, etc. that were not easily removed without adversely affecting the content or quality of the software or documentation. This documentation is intended to apply to the VOE software only. The VOE is intended for use in a non-VA ambulatory care facility.

The VistA FOIA and, therefore, VOE, is an extremely robust and complex system. This document is not intended as a full and complete manual on all functionality within the distributed software. This document will highlight VOE specific software and other software packages that may prove useful to clinicians in a non-VA ambulatory care setting. Full documentation for all original VistA FOIA functionality can be obtained via the VA website.

WARNING: This is a beta version of VistA-Office EHR, and absolutely not to be used for production!

Server/Caché Install

INSTALLATION NOTES

Download Single-User Cache software from: http://www.intersystems.com/cache/downloads/

Steps from 8. to 11. do not apply in the single-user version of Cache (the one available for download on Cache website). If you wish you can contact Intersystems (http://www.intersystems.com/contact/index.html) and request a free multi-user license and a CD for evaluation purposes only (not for production environment). This will allow you to have a fully functional Vista-Office.


END INSTALLATION NOTES


1. Read the Cache Procurement Guide.doc located on the VistA-Office EHR CD in the Install folder. Once you have obtained your Caché CD from InterSystems insert the CD for Caché in the CD drive.

2. Determine where the Caché will be installed, a server that all users can access is preferable.

3. Click the Install button, and then click Yes.

4. Accept Cache as the installation name by clicking Next.

5. Accept Standard setup, then click Next. You may change the Destination Folder by clicking Browse and choosing the appropriate folder.

6. Accept 8-bit and click Next.

7. Click Next at the Install Summary screen.

8. An Enter License Prompt will appear:

9. Click Yes, then click Next at the License Wizard Introduction to get to the Set License Information screen.

10. Enter your License Information as provided by InterSystems – in a pamphlet in the inside sleeve of the Caché CD case. (NOTE: You must enter License Information exactly).

11. Click Next, then click Finish once you have entered the License Information correctly. (Note: You will see a message that Caché is searching for previously installed components. A progress bar for this process will appear at the bottom left of your screen).

12. It will then say, Starting Caché.

13. Un-check the box in the Setup Complete screen next to the question “Do you wish to view the Getting Started page?” Click Finish, and then click Exit.

14. Remove the CD and store in a safe place!



VistA-Office EHR Install

INSTALLATION NOTES

Download a beta version of the Vista-Office software at http://sourceforge.net/projects/canfloss/

Steps from 8. to 11. do not apply in the single-user version of Cache. This only means that the install process is more streamlined.

END INSTALLATION NOTES


1. Insert the VistA-Office EHR CD into the CD drive.

2. Create a new folder in the CacheSys directory in the Mgr folder (located on the C: drive or the destination you choose in step 5 in the previous section). Name the new folder EHR. Hit Enter.

a. Go to your C: drive.

     i.	One-way to get to your C: drive is to double-click on the My Computer icon on you’re desktop, then choose C: Drive in the Address box.  Note: If you chose a drive other than the C: Drive in step 5 on page 1, then you should open that same Destination folder instead of the C: Drive here.

b. Double-click on CacheSys Folder.

c. Double-click on the Mgr Folder.

d. Go to File, click on New, and then click Folder.

e. Type EHR for the folder’s name, and then hit Enter.

3. Then go to your CD drive from the window you currently have open. To do this, click on the arrow to the right of the Address box at the top of your screen. Choose the appropriate drive (e.g. D: drive).

4. Locate the VOE_18July05.exe file located in the Install folder on the CD.

a. Double -click the VOE_18July05.exe file.

b. The WinZip Self Extractor window opens.

c. Click the Browse button to extract the documents to C:\CacheSys\Mgr\EHR by double-clicking on the C: drive (if that is where you have created the CacheSys directory), then double-click on the CacheSys directory, double-click on the Mgr folder, and then double-click on the EHR folder. Click OK.

d. Click Unzip. Wait a few minutes while the files extract. Click OK. Click Close.

5. Then click on the blue cube in the system tray at the bottom right of your desktop screen near the clock, then click on Configuration Manager from the list.

6. Click on the Databases tab within the Caché Configuration Manager box.

7. Click on Add to add a new database, and call it EHR when prompted.

8. Click Next.

9. Choose the directory where EHR is (e.g. C:\Cachesys\Mgr\EHR) by double-clicking on Cachesys (or the directory), double-click on the Mgr folder, then double-click on the EHR folder.

10. Click Next.

11. Click Yes on the next box, and then click Finish.

12. Next, you need to create a Namespace, so click on the Namespaces tab at the top of the Caché Configuration Manager screen.

13. Click the Add button.

14. Add EHR as a Namespace (as seen above), the click Next.

15. Click on the EHR database (seen below):

16. Click Next and Finish.

17. Double-click on the EHR namespace, as seen below:

18. Click on Global Mapping and then click the Add button. Type %Z* (CASE SENSITIVE) and then click OK.

19. Click on Routine Mapping and then click the Add button to add the following seven entries (also seen in the picture below):

20. Click OK to save new Namespaces (above).

21. Then click Activate in the next box (seen below):


22. Caché and EHR are installed.

23. Click on the blue cube in the system tray (near the clock located at the bottom right of your desktop screen) and select Control Panel from the list.


INSTALLATION NOTES

Please perform steps 24-28 exactly as described, but keep in mind that they will have no effect: the single-user version of Cache lets you create other users, but they are nothing more than aliases for the single user that the system accepts. The password you enter will not have any effect. This is one of the most troublesome issues that we need assistance from Intersys with.

24. Double-click on Security folder located on the left of your Control Panel screen. Click on the User Accounts folder.

25. Right click on the TRM user, select Edit, and change the namespace to EHR. Click OK.

26. Right-click on the User Accounts folder and select New User.

27. Select an Account Name (not case-sensitive) and Password (case-sensitive) twice.

28. Change the Namespace to “EHR” by choosing EHR from the drop-down list. Then change the Routine to “^ZU” then click OK. Close Control Panel screen.

INSTALLATION NOTES

The next step is completely wrong for a local installation: the server address is 127.0.0.1.

29. Determine the IP Address of the machine (preferably a server) and notate this for Configuration for the client machines.

a. To determine the IP Address of your machine, click on the Start Menu at the bottom left of your desktop screen.

b. Then, click on Run. Type cmd in the Run box and then click OK.

c. You will see a screen pop up with C:\> in it. Type ipconfig after C:\>, and then hit your Enter key.

d. Your IP Address should be listed (IP Address: xx.xxx.xxx.xxx).

e. Write down your IP Address (xx.xxx.xxx.xxx) and save it for a step in the Client Install. (Note: This number will be referred to as the [site ip] later in this document).

f. Close screen.




CPRS Install

1. Install the CPRS directory and shortcuts to all the computers that will access the EHR.

2. Create a new folder on the C: drive. Name the new folder ehrgui. Hit Enter.

a. Go to your C: drive. One-way to get to your C: drive is to double-click on the My Computer icon on you’re desktop, then choose C: Drive in the Address box.

b. Go to File, click on New, and then click Folder.

c. Type ehrgui for the folder’s name, and then hit Enter.

3. Unzip the VOECPRSGUI.zip directory (located in the Install folder on the VistA-Office EHR CD) and extract its contents to the ehrgui folder on the C: drive, the directory includes a file “CPRSChart.exe”.

a. To unzip this file, double-click on ehrgui.zip from the CD, and then click on the Extract icon from the top of the Zip screen that pops up. Choose the C: drive as the location to Extract to: by clicking on the C: drive. Then, click Extract all.

4. Locate CPRSChart.exe file - located where the files from step 2a were extracted e.g. the C: drive. Right-click on the file and choose “Create Shortcut.” Drag the Shortcut to CPRSChart.exe file to your desktop.

INSTALLATION NOTES

Step 1. above can be a little confusing. It should read something like “Perform the following steps”, and nothing more.

On the next step, your IP address should be 127.0.0.1. So, following the example below, the Shortcut for CPRSChart should read: “C:\ehrgui\CPRSChart.exe s=127.0.0.1 p=9210”

5. Right-click on the “CPRSChart” shortcut icon located on your desktop, and then select Properties. Go to the Shortcut tab once you are in the Properties dialog box. The Target will be “C:\ehrgui\CPRSChart.exe.” Place your cursor at the end of this text and type the following: “s=your site ip address here p=9210.” The s= needs to be reset to the IP address of the server that was determined in step 29 of the previous section. The final target should look something like this: “C:\ehrgui\CPRSChart.exe s=xx.xxx.xxx.xxx p=9210”.

6. Hit OK when you are finished changing [site ip] to the correct ip address.


Roll-and-Scroll Install

This section does not work, at least until we have information from Intersys on what that single-user, unchangeable password is.

1. Open a terminal emulator (HyperTerminal will work).

a. To open a HyperTerminal, click on Start at the bottom left of your desktop screen. Then, click on Programs, Accessories, Communications, and then click HyperTerminal.

b. If you have created and saved a VistA-Office HyperTerminal from a previous session, then click on Programs, Accessories, Communications, then click on the HyperTerminal folder which should contain your saved HyperTerminal Session.

2. Next you will need to type your Area Code in the Location Information screen that pops up. You do not need to fill out any of the information in the Location Information box. Click OK after you have entered your Area Code, then click OK on the Phone and Modem Options screen.

3. Type VistA-Office as the Name in the Connection Description box that opens, then click OK.

4. Choose TCP/IP from the drop-down list as the Connect using. Type your IP address determined in step 29 d & e (in the previous section of this document) in the Host address space located in the Connect To screen. Click OK.

5. The screen shot below should appear on your screen if you have connected properly.

6. Type the Username and Password as determined in step 26-27 of this guide. When typing the password, you will not see characters appear on your screen indicating that you are typing. Go ahead and type in your password when prompted, then hit Enter.

7. When you get to the ACCESS and VERIFY code Prompts, enter the following for each:

a. ACCESS CODE: start123

b. VERIFY CODE: manage#1

8. To Start the background jobs, use the following procedure (this MUST be done each time the VOE system is restarted!!!). At the first Prompt, “Select Systems Manager Menu Option:”

11. First, change the name to a real name. After the name is changed, hit the down arrow key on your keyboard until you reach the bottom of the screen and “Command: Close” appears. Then press Enter.

12. Scroll down to “Want to edit ACCESS CODE” then enter “Y.” DO NOT HIT ENTER! Instead, hit the down arrow once and it will Prompt you to Enter a new ACCESS CODE. Type in an Access code that you will remember (i.e. doctor1, name123, etc.).

a. NOTE: Do not enter a VERIFY CODE – you have deleted the Verify code for the existing client by entering a new Access code. You will be prompted to enter a Verify code at your next VistA-Office CPRS login.

13. Next, hit the down arrow until you reach the bottom of the screen where you see the “COMMAND:” Prompt. Type “Save” then hit Enter.

a. To change or add more doctors (doctor,two; doctor,three; etc.), nurses (nurse,one; nurse,two; etc.), and clerks (clerk,one; clerk,two; etc.): type “exit” in the COMMAND space, then hit Enter. Follow the instructions from step 11 until you reach this point again to add another doctor/nurse/clerk.

14. If you are finished adding and changing users type “Exit” in the “COMMAND:” Prompt and hit Enter.

15. Close the HyperTerminal window once all users have been saved and you have typed “Exit” at the “COMMAND:” Prompt. If you receive a message asking if you would like to save the HyperTerminal session for future use, click “Yes.” If you need to add or edit users and you have saved your HyperTerminal session, then start at step 1b and you will skip from step 1b to step 5 of the Client Install for Roll-and-Scroll instructions.

a. To open the CPRSChart software file, click on the shortcut that was created on your desktop in steps 4 and 5 of the CPRS Install.

16. When the users you have added/edited actually try to connect for the first time using telnet of CPRSChart, they will type their Access code (as assigned) and then their Verify code will need to be left blank. After the Access code has been entered, the system will Prompt them to change their Verify code at their first login.

17. Go to the Documentation folder on your VistA-Office CD and print the documents titled “Helpful Hints_R&S.doc” and “VistA-Office CPRS Instructional Handout.doc.” Follow the helpful hints to set up electronic signature codes for your users on the roll-and-scroll side of the system plus review the instructional handout to use the VistA-Office CPRS software.

18. The Clinical Coordinator user is the Super User for the system, please rename this user appropriately. This user also has an option called DOQ-IT setup, to get to this menu have this user type DOQ at any menu option.

Cache Terminal (Access from Server)

INSTALLATION NOTES

This section does not accomplish what it says, most notably “Starting Task Manager”. You can perform these tasks, to get acquainted with the look-and-feel of the Cache Terminal, but aside from the user Configuration part (from step 17 to the end), it is simply useless. Please perform steps 23 and following, up to the end of the section. Please scroll down to the end, for the “real” startup steps.

END INSTALLATION NOTES

NOTE: you will receive several warning messages that state “WARNING -- TASK MANAGER DOESN'T SEEM TO BE RUNNING!!!!” One of the purposes of these instructions is to start TASK MANAGER. Please ignore this warning.

WARNING: This method of entry to VOE provides a very, very deep level of access to the system. If the user does ANYTHING other than following these directions EXACTLY, it will most likely damage the Cache and/or VOE installation beyond repair.


1. Right-click on the Cache blue cube at the right bottom of the screen in the system icon tray.

2. Select Terminal from the pop up menu to bring up the Cache Terminal window.

3. At the EHR> Prompt, enter “D ^XUP” to enter FileMan.

4. At the Access Code Prompt, enter “start123”.

5. At the Select OPTION NAME: Prompt, enter “EVE”.

6. The system will display a list of options, enter “1” to select the first option.

7. At the Select Systems Manager Menu Option: Prompt, enter “TA”

8. At the Select Taskman Management Option: Prompt, enter “TA”

9. At the Select Taskman Management Utilities Option: Prompt, enter “REST”

10. At the ARE YOU SURE YOU WANT TO RESTART TASKMAN? NO// Prompt, enter “Y” to restart Taskman.

11. Hit “enter” twice to return to the Select Systems Manager Menu Option: Prompt

12. At the Select Systems Manager Menu Option: Prompt, enter “OP”

21. Next, hit the down arrow until you reach the bottom of the screen where you see the “COMMAND:” Prompt. Type “Save” then hit Enter. a. To change or add more doctors (doctor,two; doctor,three; etc.), nurses (nurse,one; nurse,two; etc.), and clerks (clerk,one; clerk,two; etc.): type “exit” in the COMMAND space, then hit Enter. Follow the instructions from step 18 until you reach this point again to add another doctor/nurse/clerk.

22. If you are finished adding and changing users type “Exit” in the “COMMAND:” Prompt and hit Enter.

23. When the users you have added/edited actually try to connect for the first time using telnet of CPRSChart, they will type their Access code (as assigned) and then their Verify code will need to be left blank. After the Access code has been entered, the system will Prompt them to change their Verify code at their first login.

24. Go to the Documentation folder on your VistA-Office CD and print the documents titled “Helpful Hints_R&S.doc” and “VistA-Office CPRS Instructional Handout.doc.” Follow the helpful hints to set up electronic signature codes for your users on the roll-and-scroll side of the system plus review the instructional handout to use the VistA-Office CPRS software.

25. The final step is to create a new folder “EHRPHARM” on the “C” drive, then create two new folders within the “EHRPHARM” folder titled FAX and HL7. The Electronic Data Interchange (EDI) files that will be sent to the pharmacies via these two methods.


25. After you create the main Fax and HL7 folders, you will also want to create sub-folders for the files that have been sent via Fax or EDI transmission.




VOE System Shutdown Procedures

1. Log onto the VOE system much like you did in the VOE Startup procedures (HyperTerminal or Cache Terminal).

2. Once you get to the access and verify code Prompts, enter the following:

a. Access: start123

b. Verify: manage#1

3. Stop the broker listeners, Mailman and Taskman as follows (input text is in RED):

4. At this time Cache can be safely stopped using the Blue Cube on the system tray. Click on the blue cube and select Stop.

VOE STARTUP:

1. Click on the Cache blue cube in the system tray and choose “Terminal”.

2. At the EHR Prompt enter “D ^XUP”, without the quotes, case-sensitive.

3. At the “Access Code” Prompt enter “start123”.

4. If you haven’t configured users before, you can do it now, by entering “XUSERED”, and following the system Prompts, just as described in the original instructions. This step only needs to be performed once.

5. Go to your Windows directory and look for the “HOSTS” (or “hosts”, not case-sensitive), file. The most usual location is C:\Windows\system32\drivers\etc.

6. If the file does not exist, you will have to create it using a plain-text editor, like Notepad. If it does exist, check that it has an entry reading 127.0.0.1 localhost (or maybe 127.0.0.1 localhost.localdomain, these are the same, just make sure there is a space between the IP and the server name).

7. Add one more entry:

8. Return to the Cache Terminal; at the EHR Prompt enter “D Q^DI”.

12. (Note: the above steps follow exactly the instructions found at http://hardhats.org, and they seem to be the right ones. This task needs to be done only once).

13. Start Task Manager: at the EHR Prompt, enter “D ^ZTMB”.

14. Start the RPC brooker: at the EHR Prompt enter “D STRT^XWBTCP(9210)”.

15. The system will display “Starting TCP listener...”, ... “Listener started successfully.”

16. Now you can click on the “CPRSChart” shortcut to start it. The access code will be the one you have set before, but for the first login, you will have to set a new verify code.

17. Steps 13. and 14. will have to be repeated each time you restart your system.

Addendum: To restart Vista-Office after reboot:

I don't see the point of doing everything above to restart VOE:

1. Just shutdown Windows and then wait for Cache to restart.

2. Start Task Manager: at the EHR Prompt, enter “D ^ZTMB”.

3. Go to Cache, click on terminal and type D STRT^XWBTCP(9210)

4. That's it.
